And the more personal "hackings" are usually the result of trusting or giving the wrong people access to your account information. Not really hacked so much as taken advantage of.

It's more like they steal the large amount of login information and check each account, either by hand or with pre-programmed scripts, and strip them of anything of value, often npcing everything in order to get gold down to the last drop.

I could go a little more in detail, but I can't claim to know that that's how they operate; merely guessing with what I've learned in my college courses and the ways it can be applied, good or bad. The key is to remember that hacking isn't some magic do-all ability, and they are limited by a variety of things.
